What Is Premature Ejaculation? Early Ejaculation Problems Explained

Premature ejaculation (PE) — also called early ejaculation — is when ejaculation happens sooner than a person or their partner would like, often with minimal stimulation. Clinically, it’s generally defined as ejaculation occurring within around one minute of penetration, persistently, and causing distress — though what feels “too soon” is genuinely personal, and plenty of people with longer times than this still experience real distress worth addressing.

 

It’s common and treatable: estimates suggest premature ejaculation affects somewhere between 20-30% of men at some point. It’s not a reflection of masculinity, attraction, or how much you want to be there.

 

 

Types of Premature Ejaculation

Depression affects mood, body, and thinking, often together:

 

Lifelong (Primary) PE

Present from a person’s first sexual experiences, typically occurring very quickly and consistently.

Acquired (Secondary) PE

Develops later, after a period of previously typical ejaculatory control, often linked to a specific change or stressor.

Psychological Causes of Early Ejaculation

Early ejaculation problems are rarely about one single thing — common contributing factors include:

Performance Anxiety

Pressure to “last longer.”

New Relationship Anxiety

Anxiety specific to a new partner.

Early Sexual Conditioning

A learned pattern of rushing from early experiences.

General Stress

Or overstimulation.

Relationship Difficulties

Or a lack of communication about sex.

Heightened Sensitivity

Physical sensitivity or arousal responsiveness.

Infrequent Sexual Activity

Can itself lower ejaculatory control temporarily.

How to Overcome Premature Ejaculation: Treatment That Works

Two well-known behavioural techniques are often mentioned for premature ejaculation:

Stop-Start Technique

  • Stimulation is paused just before ejaculation and resumed once the urge subsides, repeated to build tolerance. (Semans, 1956)

Squeeze Technique

  • Similar, but with gentle pressure applied at the point of near-ejaculation to reduce arousal. (Masters and Johnson, 1970)

 

 

An honest note on these techniques: used entirely alone, without addressing the underlying psychological or relational factors, research suggests their benefits fade over time — one study found response rates for behavioural techniques alone dropped to around 25% by three years. They tend to work far better as part of structured psychosexual therapy that also addresses performance anxiety, relationship dynamics, or whatever’s genuinely driving the pattern.

Our approach combines psychoeducation, addressing performance anxiety directly, and — where useful — guidance on behavioural techniques like stop-start, always alongside the psychological and relational work that makes those techniques actually stick. This isn’t about a quick fix; it’s about understanding the pattern and building genuine, lasting confidence.

 

A reminder on what this doesn’t involve: in line with COSRT’s code of ethics, there is no physical examination and no physical touch between therapist and client at any point. This is entirely talk-based support.

Nariman also brings direct clinical experience from an NHS sexual health clinic and hold a Postgraduate Diploma in Psychosexual and Relationship Therapy, accredited by Middlesex University.
